The military had previously adopted a "conflict management" approach towards Gaza, misleadingly interpreting evidence since at least 2018 that indicated Hamas's intent to launch a large-scale military operation as unlikely. This perception was bolstered by Hamas’s deceptive tactics, causing complacency within Israeli ranks. The report notes a significant disconnect between intelligence assessments of Hamas and the realities on the ground, pointing to insufficient discussions surrounding contingencies for a worst-case scenario.

As the situation developed, intelligence assessments began to reflect a more immediate threat, but critical evaluations were not escalated to senior officials. The resultant complacency led to vulnerabilities, including a disconnection from understanding Hamas's cultural and ideological motivations.

The review urges reforms within military intelligence, advocating for a culture that prioritizes open debate, skepticism, and comprehensive analysis. It highlights that protection of sensitive intelligence sources may have delayed appropriate responses prior to the attack and asserts that the Gaza Division suffered severe operational setbacks during the initial hours of the assault.

In the aftermath of the report's presentation, outgoing IDF chief Lt Gen Herzi Halevi took full responsibility for the failures on the day of the attack, an acknowledgment that comes amid calls for a thorough investigative commission to prevent future incidents. In contrast, Prime Minister Benjamin Netanyahu has refrained from admitting personal accountability for the events, prompting criticism from opposition leaders.
