During a session at the New South Wales Coroner's Court, it was disclosed that Cauchi, 40, a knife enthusiast with a schizophrenia diagnosis, had ceased his medication and was living homeless at the time of the attack on April 13. The court learned that Cauchi initiated his rampage without any prior warning, fatally stabbing his first victim just three minutes after entering the Westfield shopping centre. Alarming reports also indicated that no emergency alarms were triggered until after police intervened.
The inquest is scrutinizing the alarming details of this tragedy, which has deeply affected the Australian community, particularly concerning discussions around gendered violence. Notably, 14 of the 16 victims were women, including five fatalities and a nine-month-old baby, which has raised concerns about targeted violence against women.
On the day of the incident, Cauchi's violent spree began shortly after 3:30 PM local time. After retrieving his knife in a bakery queue, he attacked a string of victims, starting with Dawn Singleton, 25, and quickly moving on to others, including Jade Young, 47, Ashlee Good, and 30-year-old security guard Faraz Tahir, who lost his life trying to save others.
Despite the rapid arrival of NSW Police, who fatally shot Cauchi less than two minutes after he began his attack, the inquiry revealed no alarm had sounded throughout the chaos, leaving many unanswered questions regarding the shopping centre’s security protocols.
Dr. Peggy Dwyer SC, assisting the coroner, explained that Cauchi's deteriorating mental health had been flagged multiple times to authorities since he had been off his medication since 2019. This investigation will investigate potential gaps in the systems that failed to provide adequate support for individuals like Cauchi, as well as assess the security measures in place at the shopping centre during the attack.
